Our Annual Membership Plan Includes ($399 Yearly)

  • 2 Dental exams
  • 2 routine cleanings and fluoride applications
  • Unlimited x-rays, including panoramic
  • 1 Emergency exam
  • Routine dental radiographs
  • 20% off additional treatment (excluding mouth guards and cosmetic procedures)
  • Add an additional family member for only $299! *$50 one-time sign-up fee

With this membership plan, there is:

  • No yearly maximums
  • No insurance deductibles
  • No claim forms to fill out
  • No claim denials
  • No insurance pre-authorizations
  • Insurance cannot deny necessary procedures
  • No pre-existing condition limitations

PROGRAM GUIDELINES

  • This is a dental discount plan and not dental insurance.
  • It cannot be used in conjunction with another dental plan or dental insurance.
  • Family members cannot be substituted for other family members.
  • Enrollees must be stated at the time of enrollment.
  • Discounts do not apply to take home whitening products.
  • Annual enrollment fee must be paid in full on or before first dental visit.
  • This plan is NON-REFUNDABLE and is for one year from acceptance date.
  • Any unused benefits cannot be carried over to another year.
  • Patient's portion of bill is due the day of service.
    Failure to pay at time of service, failed or broken appointments without a 24-hour notice, voids your plan.
  • Plan and enrollment fees are subject to change annually without notice.
  • If using CareCredit, 0% interest plan does not apply.
